Safety Analysis – FMEA, FTA, FMEDA, HAZOP, DFA

Safety analyses are among the most effective investments in product quality. Performed early, they reveal weaknesses while changes are still cheap, instead of during testing or after release. They make the failure behaviour of your system visible, help you to choose effective safety mechanisms, reduce late redesigns and warranty risk, provide the evidence expected by ISO 26262 and by your customers, and create a shared view of risks across system, hardware and software teams. FMEA – Failure Mode and Effects Analysis

The basic method for the analysis of system and hardware design. Each element is examined for the ways it can fail, the effect of the failure and how it is prevented or detected, leading to prioritised improvement actions.

FTA – Fault Tree Analysis

A top-down method that starts from an undesired event, such as the violation of a safety goal, and identifies the combinations of faults that can cause it. It shows how safety mechanisms and redundancy reduce the risk.

FMEDA – Failure Modes, Effects and Diagnostic Analysis

An extension of FMEA for hardware that adds failure rates and diagnostic coverage. It quantifies the hardware architectural metrics (single-point fault metric, latent-fault metric) and the probabilistic metric for random hardware failures required for the target ASIL.

HAZOP – Hazard and Operability Study

A structured team workshop that uses guide words to identify deviations from the intended function and the hazards they cause. It is particularly suitable for the concept phase and for the hazard analysis.

DFA – Dependent Failure Analysis

Required by ISO 26262 to demonstrate independence or freedom from interference between elements. It examines common cause and cascading failures, for example through a shared power supply, clock or software resource. All analyses are documented so that they can be used directly in your safety case and in assessments.
